  • "Managing Your Medicines" in Northern Ireland
    "Managing Your Medicines" is a pharmacy-based medication review service for vulnerable or at risk patients. Implemented by the four Health and Social Services Boards in Northern Ireland in August 2000, it is a structured and comprehensive medication review protocol.

  • New Zealand Pharmaceutical Care Plan
    The New Zealand College of Pharmacists has devised forms for use in pharmaceutical care. The forms make provision for data collection (patient history and medication summary) and the setting out of a care plan.

  • Medication Review
    Patient-centred medication review is a powerful tool in optimising pharmaceutical therapy and minimising medication-related problems.

  • Auditing Pharmaceutical Care (PDF)
    The Royal Pharmaceutical Society has prepared an audit for pharmaceutical care in order to provide a framework for pharmacists to assess the quality of their work in this field. It is designed for pharmacists providing a pharmaceutical care service or those moving into pharmaceutical care. It will also help pharmacists wishing to incorporate elements of pharmaceutical care into the services that they already offer.

  • PSNC's Resource Pack for Community Pharmacists (PDF)
    "Community pharmacy medicines management - a resource pack for community pharmacists" is available from the medicines management project website. This is a practical "how to" guide on what to do when starting up a medicines management service in a community pharmacy. It covers assessment of premises, training and development needs, what medicines management service to deliver and how to plan for and conduct the patient consultation. It includes checklists to help with assessments and contains sample forms for documenting care issues, action required and desired outcome.

  • Drug Related Problem Classification
    A system for classifying drug related problems (DRPs) can be found on the Pharmaceutical Care Network Europe site. The classification is for use in research into the nature, prevalence and incidence of DRPs and also as a process indicator in experimental studies of pharmaceutical care outcomes. It is also meant to help healthcare professionals to document DRP information in the pharmaceutical care process.
